What are the two types of functional dyspepsia?

The two types are postprandial distress syndrome (PDS) and epigastric pain syndrome (EPS). PDS is defined by meal-related symptoms: bothersome fullness after eating and getting full too early. EPS is defined by pain or burning in the upper-middle abdomen. The Rome framework separates them because they behave differently and respond to different first steps.

Functional dyspepsia is a gastroduodenal disorder of gut-brain interaction, or DGBI, which means persistent symptoms in the upper stomach region without a structural cause that shows up on testing. Nothing is torn, ulcerated, or blocked. The plumbing looks normal, but the signaling between the gut and the brain is not working the way it should. What is postprandial distress syndrome (PDS)?

PDS is the meal-related subtype. Its two defining symptoms are bothersome postprandial fullness, meaning an uncomfortable, lingering fullness after a normal-sized meal, and early satiety, meaning you feel full so quickly you cannot finish a meal you would normally eat. The common thread is that eating triggers or worsens the discomfort.

Mechanistically, PDS tends to track with two things. The first is impaired gastric accommodation. Normally the top of the stomach relaxes to make room as food arrives, like a balloon expanding smoothly. In PDS that relaxation is blunted, so a normal volume of food feels like too much. The second is hypersensitivity to meals, where the stretch and chemistry of eating register as discomfort at a lower threshold than they should. Some people with PDS also have slower stomach emptying, though emptying speed does not cleanly separate the subtypes. The practical signature of PDS is simple: the clock that matters is the one that starts when you sit down to eat.

What is epigastric pain syndrome (EPS)?

EPS is the pain-centered subtype. It is defined by bothersome pain or burning in the epigastrium, the upper-middle part of the abdomen just below the breastbone. Unlike PDS, the discomfort is not necessarily tied to meals. It can come on when the stomach is empty, and eating sometimes helps rather than hurts. The sensation is pain or heat, not fullness.

One distinction matters for sorting EPS from other problems: the pain is not relieved by passing stool or gas. Discomfort that eases after a bowel movement points toward a bowel-centered disorder rather than a gastroduodenal one, so that detail helps keep EPS in its own lane. The burning of EPS can feel a lot like heartburn, which is one reason the two get confused, and it is worth understanding why after-meal burning keeps coming back when acid alone does not explain it. Under the surface, EPS involves visceral hypersensitivity, where normal signals from the gut lining are amplified into pain, and often a low-grade inflammation in the duodenum that keeps those nerves on edge.

Can you have both?

Yes, and it is common. A large share of people with functional dyspepsia have overlap, meaning they meet the description of both subtypes at once: meal-related fullness and early satiety alongside epigastric pain or burning. The two categories are useful labels, not sealed boxes, and symptoms can shift over time from one pattern toward the other.

Overlap is not a sign that something is more seriously wrong. It reflects the fact that the underlying mechanisms, impaired accommodation, visceral hypersensitivity, low-grade duodenal inflammation, disturbed signaling along the gut-brain axis, and frequently a layer of anxiety feeding into all of it, do not respect the neat lines the categories draw. When someone has overlap, clinicians usually treat whichever set of symptoms is most bothersome first, then adjust.

Why does the subtype matter?

Because the subtype points to a different first move. EPS, the pain-and-burning picture, is more often treated first with acid suppression using a proton-pump inhibitor (PPI), a medicine that lowers stomach acid production. PDS, the meal-related picture, more often gets strategies aimed at how the stomach accommodates and moves food. Starting in the right lane saves time and trial-and-error.

The distinction is a starting point, not a rulebook. Several tools apply across both subtypes. Testing for Helicobacter pylori, a stomach bacterium that can drive dyspepsia, matters in PDS and EPS alike, and treating it when present sometimes resolves symptoms on its own. Neuromodulators, such as a tricyclic antidepressant used at a low dose for its effect on gut-brain signaling rather than for mood, are used in functional dyspepsia and are better established for the pain-predominant EPS picture. Behavioral and gut-brain therapies help across the board because they work on the signaling layer itself, not just the acid or the motility. The label tells your clinician where to begin. What to do

  1. Notice the shape of your own symptoms. Is the main problem fullness and getting full too fast, or is it pain and burning? Does eating trigger it, or does it happen regardless of meals?
  2. Track it for a couple of weeks. Jot down what you feel, when it hits, and its relationship to meals. That pattern is exactly what a clinician needs to sort PDS from EPS.
  3. Make sure alarm features have been excluded. If you have any of the red flags above, get evaluated before assuming the diagnosis is functional.
  4. Ask your clinician which subtype fits you, and what that means for your first treatment step. It is a fair and useful question.
  5. Discuss testing for H. pylori. It matters in both subtypes, and finding and treating it can change the picture.
  6. Give the first approach a fair trial and report back on what changed. Functional dyspepsia is usually managed by adjusting over time, not by getting it perfect on the first try.
